Types of Medicare Advantage Plans


Health Maintenance Organization (HMO) plans

In most HMO plans, you can only go to doctors, hospitals, or other healthcare providers in the plan's network, except in an urgent or emergency situation. Referral from your primary care doctor for tests or to see other doctors or specialists may be needed.

 

Preferred Provider Organization (PPO) plans

In a PPO plan, if you go to doctors, hospitals, and other healthcare providers that belong to the plan's network, you'll pay less. If you use doctors, hospitals, and providers outside of the network, you'll usually pay more.

 

Private Fee-for-Service (PFFS) plans

PFFS plans are similar to Original Medicare. As long as they accept the plan's payment terms, you can generally go to any doctor, other healthcare provider, or hospital. The plan determines the cost it will pay for doctors, hospitals, and other healthcare providers and the cost you must pay when you get care.

 

Special Needs Plans (SNPs)

SNPs offer focused and specialized healthcare for specific groups of people, such as those living in a nursing home, having certain chronic medical conditions, or having both Medicare and Medicaid.

 

HMO Point-of-Service (HMO-POS) plans

A HMO-POS plan may allow you to get some services out of the plan's network for a higher copayment or coinsurance.
 

Medicare Advantage Plan Eligibility
 

Those who have Medicare Parts A and B and live in the plan's service area are eligible to join. Generally, people with end-stage renal disease (permanent kidney failure) can't join a Medicare Advantage plan.
 

Cost of Medicare Advantage Plans
 

You'll usually pay one monthly premium for the services included in a Medicare Advantage plan in addition to your Part B premium. It's important to compare plans in your area and understand plan costs and benefits before you join as each Medicare Advantage plan has different costs for services and different premiums. 
 

Medicare Advantage Plan Coverage
 

A Medicare Advantage plan covers all of the services that Original Medicare covers, except hospice care. Even if you're in a Medicare Advantage plan, Original Medicare covers hospice care. You'll be always covered for emergency care in all types of Medicare Advantage plans. Medicare Advantage plans will offer emergency coverage outside of the plan's service area (but not outside the United States).

 

Extra benefits such as dental care, eyeglasses, or wellness programs are also offered by many Medicare Advantage plans. Medicare prescription drug coverage (Part D) is included in most Medicare Advantage plans.

 

You'll pay one monthly premium for the plan's medical and prescription drug coverage in addition to your Part B premium. Benefits of the plan can change from year to year. You will want to make sure you understand how a plan works before you join.

Medicare Advantage Plans in Detail


Get Medicare benefits through Original Medicare or a Medicare Advantage Plan (such as an HMO or a PPO). The government pays for Medicare benefits if you have Original Medicare. What Do These Drug Plans Cover


Medicare drug plans have different tiers for drugs based on their formularies. Drugs in these tiers have different costs. A drug plan can make changes to its formularies during the year within the guidelines set by Medicare. If a drug that you're taking is part of this change then your plan needs to do one of these:
 

  • Written notice must be provided to you at least 60 days prior to the date the change becomes effective

  • When you request a refill, provide written notice of the change and a 60-day supply of the drug under the same plan rules as before the change
